Dear medical students, residents, fellows, staffs, and others:
This fascinating CPC case is a Thai female patient who presented with a 8-month course of chronic intermittent syndrome (comprising  dry cough, dyspnea, and fever in some episodes), initial weight loss, and interesting CXR/CT findings.
Every one especially Aj DK is very much welcome to make comments or discussion krab! The one who is able to make the most correct Dx with reasonable explanation will receive the reward from the head of our department.

ขอ CT หลังไมค์ แบบจัดเต็มนะครับ
ไม่อยากเข้าป่า
เพราะดู CT แล้ว มันไม่เหมือนกับโรคที่ protocol พยายาม lead อยู่

ช่วงระหว่างเดือน มีใคร CXR คนไข้หรือไม่ครับ ผล?

น้ำ BAL ออกมาแดงเลย หรือทำ trauma แล้วมันแดง (ไม่เห็นบรรยาย hemosiderin layden macrophage เลย) เลยอยากทราบครับ

มีคนทำ transbronchial Bx หรือไม่ครับ

Dear Aj DK:
1. Pichit, would you mind to send the chest CT to Aj DK? You can have our residents who make an elective course at Ramathibodi.
2. We don't have CXR during remission period of her symptoms krub; we present the complete protocol with added valuable information, much much more than real life clinical practice since it's not fair for the discussant who does not take care the patient directly.
3. No, BAL specimens were not in red color; the first one contained only alveolar macrophages and mixed inflammatory cells with rare red cells, and the second one contained no macrophages nor respiratory epithelial.

เพิ่งกลับมาจากพาเจ้านายที่ญี่ปุ่นไปเที่ยวครับ
เดี๋ยววันนี้จะแวะเข้าไปเอา CT ครับ
อืม CXR remission ตกลงมัน remission หรือแค่ดีขึ้นนิดหน่อยก็ไม่รู้
แล้วก็ protocol ที่ให้มากกว่าชีวิตจริง บางทีมันก็ช่วย บางทีมันก็ lead ให้เราหลงทางเหมือนกัน
ดังนั้น ก็คงต้องลบ bias ออก ดูตามเนื้อผ้า ว่าทุกอย่างมันเข้าได้หรือเปล่า หรือมันขัดกัน แล้วก็ต้อง recheck เป็นระยะ

ขอบคุณสำหรับข้อมูลว่า BAL ดูใสดีครับ

Thank you again, Aj DK for your full cooperation.
We try our best to remove all data that can mislead the discussant and readers; don't afraid too much regarding that issue!!??!!
The patient's clinical condition just  partially improved during the intermission period; unluckily we don't have CXR during thiese periods!

ท่าทางกระทู้ไปเร็วครับ คนอ่าน 65 แล้ว
หลังจากได้ CT ก็ทำให้แน่ใจว่า ภาพที่เห็นใน protocol ใน CT เป็นแบบนี้ทั้งหมด
เอาเป็นว่า
ผมทราบแล้วครับ

อ ชุษ ไม่ได้พาเข้าป่าอย่างเคย
และ case นี้ ผมเคยเห็น แต่ตอนเป็นแบบน้อยๆอยู่
แบบเยอะแบบนี้ ไม่เคยเจอ
พอมั่นใจว่า ถ้า อ ชุษ ไม่พาเข้าป่า
ผมแทงแล้วครับ

ขอคนอื่นมาร่วมวงกันด้วยครับ

manual เล่มเดียวไม่พอ เอาเล่มอื่นด้วย  ;)
แล้วผมจะลาก dent ที่อื่นมาจอยด้วย

เข้าใจครับว่าหนังสือเล่มนั้นหนาอย่างกับ yellow pages
เสียดายที่ปกไม่เหลือง

เข้าเรื่องดีกว่า
ผมใบ้เพิ่มว่า
ผมคิดว่า CXR ในระหว่างเดือน มันไม่ regress หรอกครับ
ไอ้ดวงๆนั้น มันไม่หดแน่ๆ
แต่ symptom และ oxygen อาจจะดีขึ้นมั๊ง (ตาม mechanism)

และที่แน่ๆ ไอ้ดวงๆนั้น
มันไม่ใช่เลือด.....

จัดให้ครับ Hint ครับ
CXR เป็น diffuse lung lesions
CT ที่ได้รับมาแบบเต็มๆ
เป็น well defined nodules + มีส่วนที่ต้องเรียกว่า mass บางส่วน
nodules มีหลายขนาด จากใน CT
ส่วน interstitial ไม่เด่นเลย
ไม่แบ่งพรรคพวกว่า เด่นบนหรือล่างเป็นพิเศษ
(แต่รูปที่ลงใน protocol มันมีอะไรบางอย่างที่ให้ข้อมูลมากๆ ขออนุญาตไม่บอกเพิ่ม)
และที่สำคัญ ไม่เห็น calcification เยอะๆ

จากข้อมูลนี้ ก็พอ narrow down เรื่อง diffuse lung lesions ลงได้เยอะนะครับ

ถัดไป ประวัติ
Protocol ให้เหมือนกับจะบอกว่า relate กับความเป็นหญิง และ มาทุกเดือน
(แต่ CT ที่ผมได้ มันไม่ทุกเดือนนี่ครับ ช่วงหลังหายไปที่ รพ อื่นหรือเปล่า หรือเพราะรู้ Dx แล้ว เลยไม่ได้ทำอะไรเพิ่ม?)
หรือว่า Protocol หลอก
ไม่ครับ Protocol ไม่หลอก (แต่เชื่อว่าซักเพิ่มให้ เพราะคนแรกๆที่ดู ถ้าไม่รู้ Dx ก็คงไม่ถามจนได้ information ขนาดนี้)
แต่บางที มันก็ทำให้เราเขวไปเป็นอย่างอื่น ซึ่ง CT ก็ฉุดเราไว้ ไม่ให้เป็นอย่างอื่น
สรุป ก็ต้องคิดว่า มันเกี่ยวกับความเป็นหญิงหรือเปล่า  ???

สุดท้าย อ่านให้ละเอียด ทุกบรรทัดมีความหมาย (คนเขียน เขียนได้เนียนนนนนนมาก)

เอาล่ะ Hint แค่นี้ก่อนละกันครับ  :D
