This fascinating CPC case is a 67-year-old woman who presented with a 2-month course of runny nose with occasionally blood-tinged discharge, hoarseness of voice, skin lesions over palms, low-grade fever, weight loss of 6 kg, and eventually hemoptysis; in accompanying with rapidly worsening of azotemia and anemia, proteinuria, positive cryoglobulinemia and direct Coombs' test, abnormal chest X-ray, and one positive AFB in the sputum. She also had preexisting diabetes, hypertension, and dyslipidemia.
