Dear all students, residents, fellows, staffs, and others:
This fascinating CPC case is a 24-year-old man who presented with a 2-week course of ascending fashion of symmetrical numbness of lower and upper limbs and ataxia, in accompanying with impairment of pin prick and propioceptive sensations and some cerebellar functions on physical examination, and abnormal cranial MRI findings.
Every one is very much welcome to make discussion and/or comments na krub ???
We still have the award for the first one who can give the most correct answer with appropriate explanation ::)

Search แล้วเจอแต่ Alcoholic Neuropathy ค่ะ แต่ไม่น่าใช่ มันน่าจะเป็น Heavy metal สักตัวปะคะ ??? (จากคำใบ้ :D)
Vit B 12 สูงจัง  ??
Serum lactate/pyruvate ก็สูง ->Heavy metal ตัวนี้น่าจะทำให้เกิด acidosis  :(

หนูสงสัยว่า Cerebellar  Signs positive หมด ยกเว้น No dysdiadokokinesia มันมีความหมายปะไรแฝงอยู่ไหมคะ

Thank you na krab
Dysdiadokokinesia is caused by cerebellar hemisphere and/or posterior lobes of cerebellum; according to the imagings most of the cranial lesions confined to midline and periaqueductal areas of the posterior fossa especially the grey matters (several nuclei).
Accompanying with these findings with ascending fashion of symmetrical numbness of all limbs, all are pivotal clues in this fascinating patient ???

Another clue: what does the cranial lesions involving the grey matters around aque duct implicate ???
You've all known this classic syndrome and the etiology::)

Serum levels of vitamin B-12 were positively correlated with the level of serum mercury.  This factor was measured only in the 1968-69 study.  Vitamin B-12 is actively involved in the metabolism of mercury, and the authors assumed that this fact accounts for this correlation.  :(

Dear Dr Kacakra:
I'm not an expert in Toxicology; I'm wondering whether Hg is neurotoxic agent due to its inhibition of myelin synthesis and SAM's action (which results in an increased catecholamine production and effects). Most of CNS involvement in this CPC patient locates at grey matters??
