ถ้าคิดเป็นเรื่องเดียวกัน น่าจะเป็น multiorgan involvement (GI, kidney)
wt loss ไม่specific แต่ course ค่อนข้างนาน ไม่น่าจะเป็น malignancy, infection ก็ดูค่อนข้างนานไป
คงคิดถึง connective tissue disease eg. vasculitis (GI vasculitis, glomerulonephritis) primary, secondary
DDx HSP->อายุมมากไป, ประวัติปวดข้อ, purpura?
       cryoglobulinemia->มี clue ว่ามี GI bleed เวลาอากาศเย็น ไม่แน่ใจว่าจะเกิดในโรคนี้ได้?(ไม่รู้ว่ายังมีโรคอื่นที่สัมพันธ์กับอากาศเย็นอีก?) เห็นเขียนว่ามี ปวดเมื่อยที่สัมพันธ์กับอากาศเย็น, ประวัติปวดข้อ, กล้ามเนื้อ?, purpura?
       SLE->น่าจะมี clinical manifestation ได้หลายรูปแบบ

However,
1. 33-kg weight loss within a year is very high, isn't it!?!
2. Most patients with essential mixed cryoglobulinemia have low levels of C1, C4, and C3; this case also has negative rheumatoid factor (what does that mean??)
3. Yes, an important clue in this case: abdominal pain and both upper and lower GI bleeding aggravated by cold; what does that mean?
4. The course is very long, about a year: it looks benign by natural course. However, a very significant weight loss seems to favor a grave etiology

และแล้วก็เข้าอีหรอบเดิมคือไม่มีใครมา discuss อยู่ดี ไม่รู้เพราะหยุดยาว หรือสถานการณ์บ้านเมืองมันวุ่นวายจนไม่มีเวลาเข้ามาเขียนตอบ เอาเถอะปลงแล้วล่ะ ไม่รู้จะมานั่งเขียน discuss อยู่ทำไม หรือจะมากระตุ้นอยู่ทำไม ในเมื่อมี participation น้อยขนาดนี้ ไม่ใช่หน้าที่ผมซะหน่อย
