This fascinating CPC case is a 30-year-old man who presented with progressive additive skin/subcutaneous necrotic lesions for 3 months and fever for 2 weeks. He also had history of chronic paranasal sinusitis of all sinuses especially right maxillary sinus for 2.5 years as well as nephritonephritic syndrome (renal biopsy exhibited diffuse mesangial & focal segmental endocapillary GN with some cellular/fibrocellular crescent) with LCV for 1 year, with partial response to cyclophosphamide, prednisolone, & enalapril.
