ตั้งแต่นาทีที่ 25 เศษๆ เป็นต้นไป ไม่ได้ยินเสียง มีแต่ภาพ ท่านอื่นพบเหมือนผมไหมครับ

ผมแจ้ง CAI คณะแล้ว เผื่อเป็นอะไรที่แก้ไขได้ แต่โอกาสคงไม่มากนัก

This patient had axonal polyneuropathy with a very prolonged course for 3 years. The electrodiagnostic test demonstrated axonal polyneuropathy, nerve entrapment, and possibly a concomitant with mononeuropathy multiplex. As she had very slowly progressive lymphadenopathies and splenomegaly without constitutional symptoms, the subtype of low grade lymphomas producing paraproteinemia including monoclonal gammopathy as well as cryoglobulinemia and amyloidosis was suspected. In this case, indolent mature B-cell neoplasms which have their postulated origin from memory B-cells with plasmacytic differentiation were my differential diagnosis. This included lymphoplasmacytic lymphoma (LPL), marginal zone lymphoma (MZL), and small lymphoplasmacytic lymphoma (SLL).
LPL is a prototype of B-cell lymphoma producing M-protein. Its presentations include cytopenia, lymphadenopathy, splenomegaly, and B-symptoms due to tumor infiltration, while neuropathy, cryoglobulinemia, amyloidosis, hyperviscosity (IgM subtype), and cold agglutinin (IgM) are due to paraproteinemia. Tumor cells are usually in bone marrow, and are sometimes found in lymphnodes (either visceral or peripheral) and spleen. Therefore, the patients presentations are fit to clinical syndromes of LPL. However, this patient presented with axonal polyneuropathy, IgG is more common than IgM as a cause of paraproteinemia-associated axonopathy. Therefore, my diagnosis is LPL producing IgG-associated axonal polyneuropathy. This patient also had cryoglobulinemia which might explain the feature of a concomitant mononeuropathy multiplex from the electrodiagnostic test.

ทาง CAI ตอบกลับมาว่า ไฟล์ที่ uploaded ขึ้นเว็บ มีปัญหาจริง แต่ไฟล์ต้นฉบับจริงๆ แล้ว มีเสียงตามปกติ และกำลังตรวจสอบแก้ไขให้เราอยู่นะครับ
