Dear all:
This fascinating very old female patient who presented with pituitary mass, lung mass, and thyroid nodule. First of all, the consultant did not take a good history of recent surgery of lung mass and poor physical examination for thyroid nodule. Hence, this case illustrates the importance of pertinent data regarding history taking and physical signs.

Aj Sarat has made an excellent approach for a correct diagnosis in an excellent fashion with a little (?) criticizing on the details of the protocol. The patient is finally received a diagnosis of MEN1 (pituitary adenoma or carcinoid, bronchial carcinoid tumor, primary hyperparathyroidism, and pancreatic tumor). A take home message from this case for all doctors is a careful history taking (a very recent history of lung surgery and a very remote history of abdominal surgery of pancreas were missed by caring clinicians) and a correlation of these data like several pieces of puzzles for making a correct diagnosis.
