This fascinating CPC case is a 57-year-old Thai male patient with diabetes and hypertension who presented with a 4-month course of bowel habit change 1 year PTA, watery diarrhea 8 months PTA, orthostatic hypotension likely due to autonomic dysfunction 6 months PTA, numbness of all extremities 1 month PTA, and significant weight loss; in accompanying with hyporeflexic paraparesis, hepatosplenomegaly, osteolytic lesions, anemia with hypocellular marrow, and diastolic LV dysfunction.
