ได้ CT มาแล้ว ขอบคุณคุณ Panpan มากครับ
ความจริงแล้ว แม้ว่าจะมีคนเคยพูดว่า CT สามารถบอก activity ของ TB ได้
แต่สำหรับผม ถ้า TB เคย Px มาแล้ว ผมคิดว่าจะให้ clear cut ว่า active จาก film คงยาก
คงต้องพึ่ง film เก่าเปรียบเทียบ
อย่างไรก็ตาม
CT ที่ได้มา เห็นเป็น consolidation ที่เนื้อข้างใน inhomogenous พอสมควร
เหมือนมี necrosis ปนๆ
จึงคิดว่าทั้ง LUL และ RLL น่าจะยัง active
ส่วน LLL มัน turn bronchiectasis ไปหมดแล้ว  แต่จะมีใครไป colonize ก็ไม่แปลก
สรุป จาก CT ที่ได้มา ผมก็ยังเอา TB อยู่ดี
DDx อื่นๆ ตามที่ discuss ไป ไม่มีอะไรมากไปกว่าเดิม

ส่วนที่ อ ประสิทธิ์ พยายามจะให้ผมคิดว่าเป็น 2 เรื่อง
ผมก็ยังต้องตอบไปก่อนว่า มันอธิบายด้วยเรื่องเดียวได้
(ตาม file ที่ต้องไป load เอา ของ case ที่ลงใน CID ที่ผมแปะไว้ใน ment หน้า 1)
ดังนั้น ผมก็ยังเอาโรคเดียวก่อน จนกว่าจะมีหลักฐานว่าไม่ใช่
แต่หาก TB ไม่เจอ แต่ผล patho มี granulomatous vasculitis ผมก็ยอมรับครับ ว่าเป็น vasculitis
(อีกครั้ง vasculitis ไม่น่ามี tree in buds อย่างไรก็ตาม CT ที่ผมได้มา ไม่ได้ส่ง lung window มาให้  อาศัยอ่านจาก protocol  นอกจากนี้ ANCA ก็ดันเป็น p ซึ่งหากเป็น c ผมคงเขวไปหา vasculitis เยอะกว่านี้)

อีกครั้ง ผมก็ยังไม่เอา crypto ครับ แม้ว่า pretest likelyhood จะเยอะ (เพราะเป็น case อ ชุษ เอามา  :D)

สรุป
ผมไม่รู้สึกว่าจะหน้าแตกหากออกมาเป็นอย่างอื่น
เพราะเหล่านั้น ต้องมีอะไรยืนยัน (ย้ำ ไม่ใช่แค่ serology เพราะมันมีของปลอมเยอะ)
ในขณะที่สิ่งที่ผม DDx ผมก็ยังต้องการสิ่งยืนยันอยู่ดี
โดยเอาจาก microbiology, cytology หรือ histology
พร้อมหน้าแตกคร้าบบบ  8)

1. How you can inform us to determine whether that the lung lesions especially 3 mass-like lesions at LUL, lingular and RLL are active?
ตอบแล้วครับ ส่วนตัว มัน inhomogenous และเหมือนจะ necrosis พอควร ผมว่า active
การมี tree in bud แล้วบอกว่า active ส่วนตัวผม ผมไม่เชื่อครับ
2. Apicoposterior lesion; is it old TB?
ผมว่าไม่ old ครับ ทั้ง LUL apicoposterior, LUL lingular และ RLL น่าจะ anterior นะครับ
3. Are these 3 lesions originated from lung parenchyma or bronchi???
งงครับ ไม่เข้าใจคำถาม
มันอาจจะ bronchiole หรือ alveoli ก็ได้ครับ
4. The patient was Dx pulmonary TB; is it related to that???
ผมก็ยังเอา TB อธิบายทั้งหมดอยู่ดี
5. The patient had extensive bronchiectais of LLL. Is it related to all 3 above mentioned lesions???
อืม อ จะบอกว่าเป็น bronchiectasis ของ LLL ไม่ได้เกิดจาก TB หรือ
ก็อาจเป็นได้ แต่ผมก็ยังคิดถึงโรคเดียวก่อน
และอาจจะเป็นของเก่า  และเกิดข้างเดียว คงไม่ใช่ autoimmune

Since at least there are extensive cystic and traction bronchiectasis of LLL and RML, respectively in accompanying with other radiologic signs of chronic inactive process (like pleural thickening of both apical and superior segments of upper and lower lobes, respectively etc), I wonder these 3 mass-like lesions are in the same process especially there are adjacent areas of tree-in-buds opacities????

เพื่อความกระจ่างด้วยคน ผมไม่ได้บอกว่าน่าจะเป็นสองโรคอย่างที่อาจารย์ DK บอกนะครับ แค่บอกว่า ก็อาจจะเป็นสองโรคอย่างที่อาจารย์ชุษบอกไว้ แต่ก็ต้องคิดว่ามันเป็นอันเดียวกันให้ได้ทั้งหมด ซึ่ง TB ก็ทำได้ อย่างที่อาจารย์ DK บอกนั่นเองแต่สุดท้ายก็ต้องจับเชื้อให้ได้อยู่ดี หรือ จะต้องเอาเนื้อมาตรวจถ้าเป็น WG แต่ถ้าที่สุดแล้ว มันออกมาเป็น Purulent infection ส่วน TB เก่าแล้ว เรื่อง neuropathy ก็น่าจะเป็นอีกเรื่อง ก็ต้องหาที่ลง ให้ได้กับ vasculitis ไปตามระเบียบ
ส่วนเรื่อง imaging ก็ถกเถียงกันไปตามสมควร บอก activity และ extension ได้พอประมาณ แต่คงยังไม่ถึงวินิจฉัยสุดท้าย

ประมาณว่า มันอาจจะเป็นของเก่า (ซึ่งก็ correlate กับประวัติ)
แล้วก็มี TB ใหม่เข้ามาน่ะครับ
(ผมไม่ได้ care กับประวัติที่ lead มาว่า เคยพบ TB ดื้อยา แล้วก็ต้องกินยานาน 18 เดือนในรอบที่ 2 ของ Px มากนัก)
แต่ผมไม่ค่อยมีความรู้ว่า
การ reactivate ของ TB ในรายที่เคยเป็น TB มาก่อน  จะเกิด immune มากน้อยแค่ไหน
คงต้องให้ อ ชุษ comment ครับ
เพราะนอกจาก infectious แล้ว หลายท่านอาจไม่ทราบว่า อ ก็เอก immuno เหมือนกัน  ::)

Since the patient doesn't have respiratory symptoms (just little dry cough) despite the presence of low-grade fever and weight loss. In addition, chest CT demonstrated at least chronic inactive process like bronchiectasis and pleural thickening. I wonder whether these 3 lesions are also bronchiectasis.
A reactivation of TB in the patient with old age group is generally due to an immunosuppressive state of the patient, and hence the type III hypersensitivity should not be that much except the relatively frequent occurrence of paradoxical reaction (IRIS) after anti-TB treatment even in non-AIDS patients.

มาถึงขั้นนี้
DDx ไปหมดแล้ว
ที่เหลือเดาใจ อ ชุษ (จากการ lead)
เป็น TB เก่า
ที่เหลือ เป็น vasculitis จาก autoimmune

แต่เวลา DDx ยังไง ผมก็ยัง discuss อย่างเดิม
ความเห็นส่วนตัว บ่อยครั้งที่ผมเจอ TB เต็มปอด แต่คนไข้ complaint ไอน้อยมาก
ดังนั้น ผมก็ยัง discuss อย่างเดิม  8) แต่ก็พร้อมจะยอมรับใน Dx อื่น หากจับหลักฐานได้จังๆ  :o

ว่าแต่ จะให้หนังสือผมอีกเล่มไหมครับ ในฐานะที่ปั่นกระทู้ได้ไกลขนาดนี้  :-[
จะได้ไปแจกเด็กๆครับ  :-*

Dear all:
Aj Kammant made the correct diagnosis after a very excellent discussion on this case regarding both neurology and immunology even though he's not an expert on immunology. The ones who are interested in the details would be able to view via the video ASAP.
The final Dx in this CPC case is microscopic polyarteritis/polyangiitis, one of pauci-immune small-vessel or ANCA-associated vasculitis. Aj Kammant the list of painful mononeuropathy (like vasculitis of small and medium-sized arteries etc) in this case versus painless mononeuropathy (like leprosy, diabetes etc), and then he indicated the difference between different kinds of vasculitis.
All abnormalities in the lung are in the inactive chronic process; all 3 mass-like lesions finally are bronchiectatic change. The rule that all excellent clinicians have to remember is some informations are coincidental but some are relevant. The problem is how could we determine which ones are relevant???

อย่างไรก็ตาม ก็ขอบคุณครับที่ติดตามการเข้าป่าของผม  :P
แล้วที่ปอด มีผล patho หรือเปล่าครับ ตรง LUL, RLL ที่เป็น dense consolidation  ???
รวมถึงไอ้เสมหะ greenๆ นี่ เจออะไรหรือเปล่าครับ  ???

Dear all:
All clinical specimens including sputum (2 times), BAL, transbronchial Bx were negative for AFB, mycobacterial PCR, and TB cultures.
The patient lost to follow-up 3 months after discharge from the hospital. I called him a few days ago (at least 12 months after his first presentation) and found that he was doing OK with some neurological sequalae like foot drop and weakness of handgrip.
