Dear all
This fascinating CPC case is a very old female patient who presented with a 3-year course of paraparesis and numbness of legs, in association with splenomegaly, and generalized visceral lymphadenopathy.
Every one is very much welcome to open the discussion or make comments krab.

What kind of lymphoma, HL vs NHL, and what type of HL or NHL in details since there are a lot of informations like a very long (3 years) course, isolated splenomegaly without hepatomegaly, small visceral lymphadenopathy, and asymetrical neurological involvement in very old woman???

--chronic progressive sensorimotor polyneuropathy (axonopathy)+ splenomegaly+lymphadenopathy (small)+leucopenia+hyperglobulinemia (reverse A:G) in 3 yr
--คิดถึง hemalologic malignancy มากสุด -->Lymphoproliferative disease (indolent course) --> NHL>>HL
--hematologic malignancy + polyneuropathy ที่คิด mechanism ออกมี
1 paraproteinemia
2 immune process (จาก immunedysregulation)
3 neurolymphomaosis (tumor involve neuro -->ส่วนใหญ่จะมาเป็น systemic lymphoma เลย หรือ CSF+ หรือPCNSL)--คนนี้คิดถึงน้อย
4 ??ใครคิดออกช่วยด้วย
--เนื่องจากผู้ป่วยคนนี้มี hyperglobulinemia (ไม่ได้บอกว่าเป็น monogammopathy หรือ polygammopathy) ขอคิดว่าเป็น monoclonal ในกลุ่ม
paraproteinemia ก่อนละกัน

--กลุ่ม paraproteinemia (แต่ globulin ไม่ค่อยสูงมากอ่ะ)
1 LPL ส่วนใหญ่เป็น IgM และมักเป็นdemyelinating แต่ก็สามารถเป็น IgG/IgA ได้ และ clinical อื่นๆไม่ค้านคือม้ามโต LNโตได้ leucopeniaได้ เป็นในอายุมาก แต่คนนี้ BMA+BMBX ปกติ??
2 plasma cell dyscrasia : amyloidosis --มักเป็น axonopathy เป็น lambda light cehain มีม้ามโตได้ BMปกติก็ได้ (clonal plasma cell น้อย)
ส่วนโรคกลุ่มอื่นเช่น MGUS, MM, POEM by clinical ไม่เหมือน
--ถ้าเป็น polygammopathy ก็จะเป็นพวกที่ relate กับ immune process
1 HL มี immune manifestation ได้ แต่คนนี้ clinical ไม่เหมือนมีแต่ node เล็กๆ+นานไป (ส่วนใหญ่ LN เด่น)
2 NHL : T cell --> AITL clinical ไม่เหมือนเพราะเป็น aggressive disease
          : B cell --> CLL/SLL clinical ไม่เหมือน BM ไม่เข้า, LPL ที่ไม่มี monoclonal gammopathy ก็ยังน่าเป็นได้
--อื่นๆ : castleman's disease (multicentric) --> neuropathy ได้ แต่ node น่าจะใหญ่กว่านี้เป็น disease ของ LN + ถ้า multicentric น่าจะดู aggressive กว่านี้
------>สรุปคิดถึง LPL > amyloidosis นะ (ถ้ามีผิดพลาดรบกวนแก้ให้ถูกด้วย ขอบคุณค่ะ)  :-X

Please make comments regarding the issue of asymmetrical neurological involvement and EMG findings (2 times), and try to correlate these information with the type of lymphoma as well ??!!??

  หนูไม่มั่นใจว่าหนูแปลถูกไหมว่า asymmetrical neurologic involvment ใน case นี้ --> mononeuritis multiplex > polyneuropathy และควร approach ไปทางนั้นแทน  ???

Follicular lymphoma is an indolent lymphoma and it is occasionally involved with spleen. Asymmetric polyneuropathy or mononeuritis multiplex is caused by tumor infiltrating nerves.

Asymmetrical neurological involvement means that the etiology should not mainly due to paraneoplastic syndrome or other causes resulting in symmetrical involvement.
But the DDx still includes roots (caudal equine), lumbosacral plexus, multiple nerves, and/or peripheral nerves. And the pathophydiology is axonpathy and/or demyelinating disease.
All of the clues both neurological involvement (symptoms, signs, investigations including imagings, CSF profile, and electrophysiology) and non-neurological abnormalities, as well as clinical course, constutional symptoms and demographic data will help in the Dx of disease and it's subtype.
In addition, apart from numbness and weakness, this patient had painful feet as well.

Dear all:
Aj Noppacharn has made an excellent discussion and the detailed correct diagnosis of LPL with IgG-typed paraproteinemia-associated neurological complications in this CPC case. He is excellent in both Neuromedicine and Hematology, and is one of the best mentors for every medical students, residents, fellows, and other doctors.

Could you please summarize the clinical details of this subtype that can be differentiated from other indolent or slow growing lymphoma subtypes for non/ hematologist and for all too busy (or too lazy) to view the video krub?
