อ่านดีๆนะครับ hereditary neuralgic syndrome ไม่มี hepatitis นะครับ เดี๋ยวจะงงที่อาจารย์นภชาญเขียนกัน คนไข้จะค่อนข้าง dysmorphic แต่แรกเกิดด้วยครับ อย่าจุดธูปบ่อยครับ ช่วงนี้กำลังยุ่ง

ใครนึกโรคนี้ไม่ออก ก็น่าจะพอคุ้นกับ Diabetic amyotrophy กันมากกว่านะครับ อันนั้น จะคนไข้เบาหวานผู้ชายเป็นมานานๆ ตุมไม่ดี ปวดแล้วก็กล้ามเนื้อลีบลง ส่วนใหญ่เป็นที่ขาครับ ชือพาลให้นึกถึง muscle แต่จริงๆ หลักเป็น nerve root หรือ plexus เพราะฉะนั้นเคสนี้น่าจะใช้คำว่า brachial neuropathy แต่จะบอกว่า neuralgic เพราะปวดแล้วก็ amyotrophic เพราะลีบก็โอเคครับ

เรื่อง hepatitis E สงสัยว่าที่จุฬามีแลบตรวจใช่ไหมครับ :)

อีกนิดละกัน ผมว่า เรื่อง pain กับ reflex ลด คงแยก anterior horn cell ออกไม่ได้ อย่างที่คุณรองพงศ์บอกไว้น่ะครับ

แล้วยิ่งมี motor อย่างเดียว ก็ยิ่งประหลาดในความเป็น plexus ด้วยซ้ำ แม้ว่าโรคนี้ จะ Motor เด่น คงดู electrophysiology ด้วยว่ามี sensory action potentials เป็นงัยด้วยอ่ะนะ แล้วผลมันว่ายังไงหว่า neuro ช่วยอธิบายด้วยครับ

กรณี acute anterior horn ก็คงไปว่า พวก entero visus polio หรือ on polio อะไรเทือกนั้น เออ แต่ ที่ upper extremities เด่น นี่มันเจอไหม เด๋วใครค้นเจอบอกด้วยนะครับ นึกไม่ออก

นึกง่ายๆ analogous กับ acute GBS กับ acute polio ที่ lower extremities ก็น่าจะได้ครับ
 

จริงๆ immune ก็คงเป็นอีกอันนึงเวลานึกอะไรไม่ออก หาไวรัสไม่เจอ เพราะ anway virus ก็ทำให้เกิด immune อยู่ดี

ตกลง hepatitis E จริงไหม :)

ขอเพิ่มจาก อ.ประสิทธิ์นิดนึงคือ ถ้ามีข้างซ้ายอย่างเดียว คงแยก anterior horn cell กับ plexus ไม่ได้จาก pain + hyporeflexia แต่ข้างขวามันมี component ของ pain + hyporeflexia อย่างเดียว โดยที่ไม่มี weakness เลยในขณะที่ข้างซ้ายมี weakness อย่างชัดเจน (คือ severe pain มันมาพร้อมกัน แต่ข้างนึงไม่ weak เลยแต่อีกข้าง weak มาก) รวม 2 ข้างจาก Hx + PE แล้วน่าจะเป็น plexus มากสุด + กับ component ของ root ตามที่เขียนไว้ก่อนหน้านั้น 2-3 post ว่าเป็น........+/-....... ในที่นี้ก็คือ plexus + root แหละ

ส่วนที่เขียนไว้ คือ brachial neuritis หรือ neuralgic amyotrophy ส่วนใหญ่มักจะมี neurological syndromes อย่างเดียว มีประมาณ 20-30% ที่อาจจะมีประวัติ illness อื่นมาก่อน แต่ส่วนใหญ่เป็น idiopathic ที่เขียนถึง hereditary คือแค่บอกไว้เฉยๆว่ามันมีสาเหตุที่เป็น genetics ด้วยไม่ได้เกี่ยวข้องกับ case นี้ หรือตั้งใจให้คนอ่านงงแต่อย่างใด

เห็นด้วย ครับว่านึกถึง plexus มากที่สุดครับ  ด้วยเรื่องของ pain แต่จริงๆ แล้วต้องเผื่อใจนะครับ เพราะ AHC ก็ pain ได้ ในช่วง acute weakness ก็มี disparity กับ pain ได้ และความจริง lesion จะเป็นตรง proximal root หรือ ตัว anterior horn cell เองมันก็ไม่ได้ เป๊ะขนาดนั้นนะครับ มันก็ข้ามๆ กันได้
ตัวเชื้อมันก็มี favourite sites แต่ก็ข้ามๆ กันได้เช่นกัน การวินิจฉัยอะไรจึงต้องบอกเป็น probablistic mode ไว้ก่อนนะครับ มันไม่เคยมีอะไร 100 % ใน medicine. :)

คนอ่านคงไม่งงแล้วนะครับ เรื่อง hereditary บังเอิญ ความคิดอาจารย์นภชาญไปไวกว่าที่เขียน ประโยคความเรียงมันเลยอาจทำให้สับสนได้ครับ

เอ เดี๋ยวนะเรื่องข้างเดียว ผมเริ่มจะงง จริงๆ ถ้ามีข้างเดียวก็จะ favor plexus นะ AHC มักจะเป็นสองข้างแต่ asymmetry ยกเว้นโรคที่ focal มากๆ (หายาก)
เอ แต่พอไปอ่านดูแล้วมันปวด confine ต้นแขนจังเลยนะ ยกแขน แล้วดีขึ้นอีก คล้ายๆ ปวด กล้ามเนื้อ หรือ secondary เลย ขอตัดเครื่องหมายออกสักสองสามอันเป็น
plexus>> AHC ก็แล้วกัน

First of all, I wonder Aj Kon Klai Ban is gone!?! But later's bettern never's!
1. Aj Tnin made a very excelent discussion regarding this case in every point you've discussed. Bilateral shoulder pain in this case was releived by a unique posture of arms; it's called "Lasegue sign" with crossing arms (flexion (of elbow)-adduction (of shoulder) posture). Hence, it prefers root/plexus, compared to horn cells.
2. The disporportion between motor power of flexor digitorum of forth and fifth fingers(innervation by ulnar nerve), and of flexor digitorum of first and second fingers (innervation by median nerve). This favors plexus as well.
3. Due to hyporeflexia of right tricep, right knee, and both ankle jerks; this also a favors plexopahty/root, compared to horn cells.
4. Why only motor is involved in plexopathy? Like Aj Prasit mentioned, this is due to a special etiology of plexopathy like GBS: autoantibody against antigen of motor part of brachial plexus. Hence, in this case the etiology must not be caused by compression or trauma or entrapment or etc.
5. There are some reported cases of acute poliomyelitis caused by Poliovirus, and Flaviviruses  (like Dengue, West Nile virus etc) involving only facial and respiratory muscles.

อ้าว neurological diagnosis เป็น plexopathy แล้ว cause o hepatitis ล่ะอาจารย์ แล้วสรุปว่า Final diagnosis คืออะไรครับ?

A final diagnosis is HEV-associated brachial plexopathy; the second case of the world. This case was published in Scand J Infect Dis 2010 by Aj Yong Poovorawan krab

I have a question: how to test these signs
