ในแง่ทฤษฎี เดี๋ยวจะงงวงแตกไปกันใหญ่ไหมล่ะเนี่ย

ประเด็นก็คือ เราทราบว่า ผลการตรวจเป็นบวก หรือ เป็นลบ แต่เราต้องการหาว่าจริงๆ แล้ว โอกาสจะเป็นโรคจะเป็นเท่าไร เพราะเราไม่มีทางทราบแน่ ว่า มันเป็นบวกจริง หรือ บวกปลอม ลบจริง หรือ ลบปลอม

สมมติเราจะหา โอกาสที่จะเป็นโรคเมื่อผลตรวจบวก (ความน่าจะเป็นแบบมีเงื่อนไข) เราจะต้องทราบก่อนว่าโอกาสที่ผลตรวจจะบวกเมื่อเป็นโรคเป็นเท่าไร แล้วก็ผลตรวจจะเป็นบวกเมื่อไม่เป็นโรคเนี่ยเป็นเท่าไร ซึ่งก็คือ sensitivity แล้วก็ 1-specificity ตามลำดับ แต่ เราจะต้องทราบความน่าจะเป็นของคนคนนี้ซะก่อนว่าน่าจะมีโอกาสเกิดโรคซักเท่าใด จึงจะใช้ทฤษฎีนี้ได้ ซึ่งอาจารย์ชุษณาใช้คำว่า prevalence แต่ในสถานการณ์จริง ก็คือ ความน่าจะเป็นที่เราคิดว่าคนไข้จะเป็นโรค ซึ่งก็มาจากหลายๆอย่างประกอบกัน จากอาการ ตรวจร่างกาย ประวัติต่างๆ หรือ แลบอื่นๆ ที่รู้อยู่ก่อนแล้ว หรือความเห็นผู้เชี่ยวชาญอะไรก็ตามที่ แล้วในทฤษฎีของตาเบยส์จึงบอกว่า โอกาสที่จะเป็นโรคเมื่อผลตรวจบวก (posterior probability)หรือที่อาจารย์ชุษณาเขียนไว้ว่า positive predictive value ก็จะหาได้จาก ผลบวกจริงหรือเซนซิติวิตี้ คูณกับ โอกาสน่าจะเป็นโรค หารด้วย ผลบวกจริง หรือเซนซิติวิตี คูณโอกาสเป็นโรค (อันนี้คือตัวตั้งนั่นเอง) บวก กับผลคูณของโอกาสที่ไม่น่าจะเป็นโรคกับ ผลบวกปลอม (หรือ หนึ่ง ลบ สเปซิฟิซิตี้) ซึ่งตัวหารนี้จะเป็นสองกรณีที่ไม่สามารถเกิดร่วมกันได้ ใช่ไหมครับ ผลบวกจริงเมื่อเป็นโรค กับผลบวกปลอมเมื่อไม่เป็นโรค จะเกิดพร้อมกันไม่ได้ แต่เราไม่รู้ว่า ของเราเป็นของจริง หรือ ของปลอม ก็เลยต้องเอาทั้งสองกรณีมาเป็นตัวหาร

เขียนแบบจริงจัง ตามทฤษฎีก็คือ p[D+|R+] = p[D]p[R+|D+]/p[D+]p[R+|D+]+p[D-]p[R+|D-]
อย่าให้ถึงกับต้องกลับไปพิสูจน์ทฤษฎีเลยนะครับ และอย่างที่บอกว่าเดี๋ยวจะงง กันไปมากกว่าเดิม ก็คือ อันที่จริงแล้วเวลาดูผลการตรวจ สิ่งที่ควรจะทราบคือ likelihood ratio ซึ่ง positive likelihood = sensitivity / 1 - specificity ทีนี้ในสมการของผมมันเป็น odd ratio คือ probability/1-probability แล้วถ้าใครว่างๆ จะลองแปลงสมการแบบออดๆเนี่ยกลับไปจะได้เป็นสมการต้นกำเนิดของเบยส์เนี่ยล่ะครับ แต่แบบออดๆ เนี่ยมันเข้าใจยากกว่าแบบความน่าจะเป็น แต่มันคำนวณง่ายกว่า

เช่นสมมติรายนี้ ความน่าจะเป็น CSSอยู่ที่ ร้อยละ เจ็ดสิบ (สมมตินะครับ แบบว่ามั่นใจมาก) จากสิ่งที่ให้มา แล้วก็ขอเจาะ พีแอนค่ามาเนี่ย ลบ LR- = 0.45 ก็จะได้ ออดก่อน เท่ากับ จุดเจ็ด หารจุดสาม คูณ จุดสี่ห้า เท่ากับ ออดแค่หนึ่ง นิดๆ จะได้ น่าจะเป็นโรคราวๆ ห้าสิบ ห้าสิบ คือยังสงสัยกำกึ่ง แต่ถ้าบวก ก็จะมั่นใจมาก เพราะ LR+ 35 แต่รายนี้ถ้าไม่ได้สงสัยอะไรมากนัก คือ พรีเทสต่ำแค่อยู่ในเชี่ยลและโรคก็เจอน้อย พอผลมาเป็นลบ ก็แทบไม่ต้องนึกถึงเลย แต่ถ้าบวก ก็ยังไม่แน่ชัดนัก


เอาง่ายๆ กว่านั้น ก็คือ การที่ผลบวกออกมาเป็นลบเป็นบวก เราไม่ได้ดูแคค่ที่ความไว หรือ ความจำเพาะของเทส แต่ต้องดูก่อนว่าเราสงสัยแค่ไหน ถ้าไม่ได้สงสัยมาก เอาแลบมา แล้วลบอาจจะช่วยเราแยกโรคไป แต่ถ้าเราสงสัยพอประมาณ แล้วเอาแลบมา แลบมาเป็นลบ จะไม่ช่วยแยกโรคจะทำให้เรางงหนักไปอีก เราควรใช้หลักการนี้อยู่ทุกเมื่อเชื่อวัน จะได้รู้ว่าจะส่งแลบอะไร ไม่ส่งอะไร แต่ที่ยากที่ดน่าจะเป็นการประเมินความน่าจะเป็นก่อนส่งแลบ อันนี้ มักไม่ได้ค่าที่ชัดเจน จะมาคำนวณกันแบบนี้ แต่นี่ก็ยกตัวอย่างการคำนวณให้เห็นเผื่อจะระลึกชาติทฤษฎีของเบยส์ได้ไม่มากก็น้อย

And what is the most likely diagnosis in this patient ie the type of ANCA-associated diseases???

เอาแบบว่ารายนี้มันดู organ น้อยไปหน่อย
มีปอด ไต +/- upper resp
ก็เอา c-ANCA ละกัน
แบบว่า ถ้า p มันก็จะยุ่งเหยิงกว่านี้ เช่น มี neuro มาด้วย  ;)

แล้วก็ที่อ่านของ อ คนไกลบ้าน
รู้สึกสรุปได้ว่า
อย่ากวาด lab ที่ไม่นึกถึง เพียงเพื่อสนองความรู้สึกที่จะแค่ R/O
เพราะถ้ามัน + มันก็ทำให้เกิดการตัดออกจาก Dx ไม่ขาด  :o
แต่ถ้า - ก็ดีใจกันใหญ่ว่าลบ  ;D

ยังอยากเห็น clinical approach มากกว่า Lab approach ใน medicine
(ยกเว้นบางหน่วย ที่ต้องใช้ lab)

ผมว่า มันก็เป็นคลินิคัล approach หมดนะครับ แม้ว่าจะเริ่มจากค่าแลบอะไรบางอย่าง เพราะต้องยอมรับว่าเดี๋ยวนี้ บางทีคนไข้ก็มาหาเราด้วย แลบจากการตรวจกรอง (จำเป็นบ้างไม่จำเป็นบ้าง) ก็ต้องมาดูอาการ ดูโอกาสน่าจะเป็น จากลักษณะทางคลินิก อิเมจจิ้ง แลบอื่นประกอบหรืออะไรก็ตาม สรุปว่าการจะได้วินิจฉัยอะไร ก็ต้องคิดหน้าคิดหลัง อย่าหว่านแลบ อย่างที่ จารย์ดีเคว่านะครับ แต่สุดท้าย ถ้าจำเป็นจะต้องใช้แลบใดฟันธง ก็ต้องส่ง อย่ามากั๊ก เดาๆ ไปเหมือนฉี่ไม่สุด

จริงๆ รายนี้ก็เหมือนเขียนพยายามจะเอา upper respiratory tract มาหลอกล่ออยู่แล้ว เพราะดูเป็นอาการธรรมด๊า ธรรมดา ในประวัติทั่วไป คงไม่เอามาใส่ แต่คนดู และคนเขียนโปรโตคอล ก็คงนึกถึงพอเห็น ปอด เห็นไต ก็ต้องถามไปให้มันทั่ว แต่ถ้าออกมาเป็น C-ANCA associated RPGN ก็เหมือนฉายหนังพลอตใกล้ๆ เดิม เพราะคราวก่อน ก็เป็น pANCA neuropathy

แต่ถ้าเอา cryo ด้วย โดยคิดว่า URI symptom มาหลอกๆ ก็ให้เป็น pulmonary renal แบบนี้ได้เหมือนกัน  CSS , micropolyangitis, HSP ก็ให้แบบนี้ได้ แต่ไม่มีอะไรมาให้นึกถึงมาก

ก็สรุปว่าเป็น pulmonary hemorrhage renal failure บวก upper tract symptom (blood stinged nasal discharge plus hoarsness) ที่เจอ เอเอฟบี มีกาแลคโตแมนแนน ขึ้น culture mycobacterium and fungus pending แต่เทใจไปให้ systemic vasculitis กันซะแล้ว ระวังแจ็คพอต จะออกที่ทีบี กะ รา

ข้อเสียของ CPC อันนึงสำหรับคนที่ดูบ่อยๆ ก็คือ อาจทำให้นึกถึงโรคพบไม่บ่อยได้บ่อย จนทำให้นึกว่าเป็นโรคที่พบบ่อย จริงๆ อยากให้ได้กระบวนการคิด มากกว่า
อย่าหลงดีใจว่าพบโรคหายากจนตื่นเต้นเกินงาม
(ยกตัวอย่าง CSS รู้สึกจะ prevalence 1:1000000 น้อยกว่าโรคพันธุกรรมมากมายแต่ทำไม๊ หมอไม่ค่อยคุ้นกันนะ เช่น มาร์แฟน เจอราว 1:10000 เฟลโลว์ คาร์ดิโอเจอตอนสอบบอร์ดยังตอบอึกอัก ไม่รู้จะ ให้คำแนะนำทางพันธุกรรมอย่างไร แต่พอvasculitis dent ก็ตอบเป็นฉากๆ และว่าจะส่งแลบนู่นนั่นนี่ โดยไม่เคยเจอคนไข้นอกจากในซีพีซีกะตำรา สงสัยต้องรีบเขียนตำรา และหาเคสเข้าซีพีซี)

ว่าแต่พยายามชักจูงผู้คนมาตอบแล้วนะครับ กำลังมา เห็นเพลินเก็บภาพซากุระบานอยู่

I wonder whether her URI symptoms especially runny nose with blood tinged discharge is usual for URI or not?????? or what's implication????

Regarding the question I asked for the type of ANCA-associated vasculitis; I mean that if the patient has this disease, what kind of it???
For example:
1. Primary vasculitis: CSS, Wagener's granulomatosis syndrome or vasculitis, MPA, PAN etc
2. Infection-associated ANCA-associated vasculitis: E. coli LAMP-1 associated vasculitis etc
3. Drug-induced ANCA-associated vasculitis
4. Other autoimmune diseases

I have a very quick look at the video and I got 'pauciimmune crescentic glomerulonephritis with pANCA and antiMPO positive' as the final answer (if it is incorrect, please correct me).

One of my concern as a clinician and also a pseudoscientist from this impressive didactic case discussion and I think because the time in the conference is limited to emphasize is 'Is it important to classify and diagnose 'vasculitis'?'
The CPC discussant said 'That might not be necessary to differentiate between MPN and WG because the treatment is not so different' That's really true in a clinical setting 'Try to classify disease should be based on clinical benefit for treatment or prognosis'

But another discussant said that this case might be secondary pauci immune vasculitis associated with infection and anti-LAMP2 might be sent at a cost about 10000 US and this discussant again said in the final part of CPC when he recognized that this case has rhinitis, but pANCA positive and so he said 'ELISA for anti-MPO and anti-PR3' MUST be sent. So is the classification of the disease in this case vasculitis only academic interest or there is any (now or future) benefits in clinical practice?

The typical quote from the practitioners is 'Go to medical school and you will receive the accurate diagnosis in your death certificate but if you want some treatment although might be just symptomatic relieve, go to general hospital'

In clinical settings, the laboratory tests or any diagnostic procedures should be performed only when they give the benefits which are out-weighted complication from that procedure; money and side effects, respectively. 8)

We, clinicians, tend to classify the patients who have similar symptoms, signs, epidemiology, risk factors and laboratory results into groups of patients that we think that they might share similar causative factors, pathogenesis and so have the therapeutic implications or will have therapeutic implications when these more homogeneous patients are subjected to clinical trials or scientific research. :)

These tedious jobs of our precedent physicians have proved themselves as scientific accuracy and also gear the new discoveries for the sake of knowledge or the benefits to the humankinds. UK just cerebrate their life expectancy estimation of the new born babies nowadays to be beyond 90 years! :-*

However, we should remember that diseases are what the human creates and they are subjected to be changed, modified, obsolete, expanded, simplified or elaborious, etc according to the knowledge and interpretation. :-\

Back to the case of vasculitis, one of the problems in classification is we don’t know exactly what is the cause or what are the minimum necessary causes for developing vasculitis. From clinical, pathological, and laboratory evidence show that clinical laboratory detection of autoantibody might play role in pathogenesis of these arrays of difficult nosological entities. (Although some are proved to be the innocent bystanders or cross reactivities with the actual culprit such as antiphospholipid antibodies) From the same sources of evidence, they show that many infectious agents may be the trigger of the diseases and molecular mimicry of the self antigen is among the famous hypothesis of the diseases.  :o

The concepts of primary vasculitis and secondary vasculitis (as elsewhere disease entities) will be ended up with less and less primary disease when the etiologic or causative agents have been recognised. In particular, in ANCA vasculitis, there are some evidences that antiMPO and antiPR3 might be some of the major players in pathogenesis of vasculitis. Recently, anti-LAMP-2 (lysosomal associated membrane protein) was found to have some pathogenetic role in ANCA-associated vasculitis. This antibody to human LAMP protein is believed to be related to fimbriated bacterial protein (although there is no prove that it really trigger the pauciimmune process.  :-[

One of the series found that more than 90% of patients with active pacui-immune FNGN have circulating anti LAMP-2 antibody while only half of patients have anti-MPO and anti-PR3 antibodies. So should the entity be considered primary and secondary? Should we abolish anti-MPO and anti-PR3 tests? Should we test anti-LAMP-2 for all ANCA associated vascultis patients instead? These questions do not have one simple answer at the moment as the sensitivity, specicificity of the tests have not been established.  :-X

Back to the topic, so in my opinion, we are in the medical school setting and we have the great opportunity to help patients to have the best case and in turn, we also have the great opportunity to learn from the patients to the very basic ground and may create some future clinical therapeutics or preventive benefits. We should encourage the learning but do not confuse the juniors (residents and fellows in training) and medical students with highly sophisticated diagnostic tests and procedures which sometimes are performed in case of research or diagnostician curiosity and sometime for epidemiological information. All the tests that are to be ordered or performed should have the goals and purposes which we should explicitly clarify and the most important thing is to overcome the curiosity of the diagnostician! >:(

In conclusion, if I will say, I might restate to ‘’MPN is a disease entity that has been proposed recently (1994 appeared in Chapel Hill classification) there might be some benefits beyond the present therapeutic implications for differentiate between these two diseases. (Can microPAN developed to WG lately?) ANCA and anti-MPO and anti-PR3 can be used for this purpose. I will still be skeptic about anti-LAMP-2 which has not been proved so far in clinical situation but if it is possible and there is payer for this test (grant bodies), it is a good candidate for the research purpose testing to verify the hypothesis (not an established! knowledge) of the bacteria and human protein molecular mimicry. (Think about anti-DNAaseB and anti-streptolysin-O in rheumatic fever, who care!- I care.55)

In the future, this classification might be obsolete when the etiologic and treatment implications are better clarification. A good example is the classification of acute leukemia from FAB classification, not very long time before, is now evolved into the classification by cytogenetics and prognosis from treatment.

Anyway, in house ELISA test can be developed which I guarantee that will be less than 10000 US.

Most of molecular diagnostics for genetic disorders or other fields can be performed with much less cost than 10000 US.

Some of the imaging diagnostic tests such as MRI with their special techniques are much more expensive than molecular diagnostic tests. :p

The final diagnosis of this CPC case should be either MPN vs  WG vs MPN/WG plus mixed cryoglobulinemia/AIHA.
I understand all thoughts with the different views from the discussants and Aj KonGlaiBan regarding it's necessary to distinguish among these 3 diseases. The first discussant consider the potential of therapeutic intervention, whereas the second concerns much regarding academic and research interest and probably difference treatment and prognosis in the future (if there are more data to support).  Another view from Aj KonGlaiBan, He concerns the humanity and learning process of Ll junior doctors and students.
My objectives to present this CPC case include a) how to approach an elderly patient with multiple comorbidities who has rapidly rising azotemia b) to update especially by an Internet search the knowledge regarding secondary pauciimmune vasculitis and c) Beyes' theorem
