อย่างไรผมก็ต้องชื่นชม ID ของเราว่า อยู่ในขั้นเทพที่สามารถ diagnosis ไอ้เจ้าตัวที่ว่านี้ได้อย่างรวดเร็วและถูกต้องจนน่าทึ่ง อันนี้ก็เป็นอีกข้อดีของการอยู่จุฬา ที่มีแต่เทพ และปรมาจารย์ในทุกๆ สาขา จะเห็นได้ว่าเรามี case report สวยๆจำนวนมากที่อ่านดูแล้วก็ต้องอุทานว่า diag ได้ยังไง  8)

ผมยังบอก resident hemato เลยว่า ดูว่าใครรับ consult ที่ตึกนี้ถ้าไม่ใช่อาจารย์ชุษณา ผมจะให้ fellow ID ปิด case นี้ไว้แล้วชงอาจารย์วันล่าให้ อาจารย์ชุษณาเป็นคนทำ แต่ก็ผิดแผนเพราะอาจารย์ชุษณารับ consult ตึกนั้นพอดี

อ.นภชาญ เป็นสมบัติอันล้ำค่าของภาควิชาจิงๆ (invaluable asset) มีความตั้งใจดี เจตนาดี และให้ความเห็นตรงไปตรงมา

สนับสนุนอย่างมากครับ ผมได้ฟังอ.ชุษ ทำ CPC ครั้งสุดท้าย (actually, ครั้งเดียว) เมื่อราวๆ 7-8 ปีก่อน  ;D

อย่างที่อ.นภชาญว่า การ discuss แบบทราบกับแบบไม่ทราบ case ต่างกันมว๊ากกกก ขอบอก

เอ้อ โอกาสหน้า ถ้าอ.นภชาญ (หรือท่านใดก็ตาม) พบเห็น case ที่น่าจะเหมาะให้ I.D. ทำ CPC แม้อ.ชุษรับปรึกษาที่ตึกนั้นในเดือนนั้น ก็มาบอกผมได้ครับ ผมจะไปเรียนปรึกษาอ.ชุษ และเกลี้ยกล่อมสุดความสามารถ ขอให้อาจารย์ยกเว้นไม่รับ consult case นั้น (ผมหรืออาจารย์ท่านอื่นดูแทน) เพื่อให้อาจารย์ได้มีโอกาสสำแดงเดชใน CPC (ทศวรรษละครั้ง)

ผมชอบอยู่โรงเรียนแพทย์อย่างจุฬาก็เพราะมี case สวยๆ rare ๆ แบบนี้ให้ได้เรียนรู้และ exercise brain ตลอดเวลาก็ต้องขอบคณอาจารย์ชุษณาที่ทุ่มเทกับ CPC ตลอดมา


อย่างจริงใจที่สุดนะครับ เห็นด้วยกับอ.นภชาญอย่างยิ่ง การทุ่มเทให้กับ CPC เป็นความดีเพียงอย่างเดียว เอ๊ย เป็นความดีที่สำคัญที่สุด ที่ผมเห็นในตัวอ.ชุษ

ผมเห็นแล้ว ทั้งชื่นชม ทั้งเหนื่อยแทน (แต่เจ้าตัวคงไม่ค่อยเหนื่อย เพราะ require การพักผ่อนวันละไม่กี่ชั่วโมงก็พอ)

ขอใช้สิทธิพาดพิงนะครับ

Hemato ได้รับ consult เพราะว่ามีคนสงสัย lymphoma ตอนไปดู case มี clues บางอย่างที่อาจทำให้สงสัย lymphoma ได้เช่น prolonged fever, weight loss, mild splenomegaly, small lymphadenopathies from imaging, and high LDH (ซึ่งก็เป็นจาก myositis ก็ได้) ส่วน DVT นั้นผมทราบตอนไปดู case ว่าไม่ใช่จาก imaging แล้ว แต่ disseminated infection โดยเฉพาะ cryptococcosis และ histoplasmosis ก็ยังอยู่ใน DDx ของผมอยู่ในตอนแรก เลยตัดสินใจให้ทำ BM study เมื่อดูจาก bone marrow aspiration แล้วไม่ได้สงสัย lymphoma เลย ตอนเจอ organism ที่ว่าก็คิดว่าน่าจะอธิบายได้ทั้งหมดจาก organism ที่ว่านี้ได้โดยตอนนั้นยังไม่รู้ว่ามันคือตัวอะไรกันแน่

ทาง ID ต่างหากที่ยังสงสัยว่าคนไข้มี underlying เป็น lymphoma ไปสั่งย้อม very extensive immunohistochemistry เพื่อพยายามหา lymphoma เองทั้งหมด ผมก็เคยพูดใน unit round แล้วว่าไม่สงสัย lymphoma เพราะ clinical มันเป็น aggressive lymphoma ซึ่ง course ที่ยาวกว่า 1 ปี มันนานเกินไป และยังคุยกับ chief ward เลยว่าน่าจะเป็นไอ้ตัวนี้ทั้งหมดนั่นแหละ ทาง ID ต่างหากที่ยังไม่ยอมหยุดจะยัดเยียดให้เป็น lymphoma ให้ได้ จน investigate หา lymphoma จนสุดทางแล้วถึงจะยอมรับว่าไม่ใช่ lymphoma แล้วค่อยพยายามมาอธิบาย clinical syndromes ทั้งหมดจากเชื้อตัวนี้
 
ผมถึงไม่เข้ามา response ตั้งแต่แรกเพราะรู้ว่า case นี้มันยากมากๆ เพราะจริงๆ ผู้เกี่ยวข้องใน case นี้ทั้งหมด ไม่ว่า Neuro, Hemato, Pulmo, ID, Rheumato ไม่มีใครสามารถอธิบาย clinical syndrome ของคนไข้ได้ทั้งหมดตั้งแต่ต้น (จริงๆตอนเจอตัวประหลาดนี้ผมยังบอก resident hemato เลยว่า ดูว่าใครรับ consult ที่ตึกนี้ถ้าไม่ใช่อาจารย์ชุษณา ผมจะให้ fellow ID ปิด case นี้ไว้แล้วชงอาจารย์วันล่าให้ อาจารย์ชุษณาเป็นคนทำ แต่ก็ผิดแผนเพราะอาจารย์ชุษณารับ consult ตึกนั้นพอดี)

จริงๆถ้าไม่ถูกพาดพิงก็คงไม่เข้ามา response เพราะรู้ case แล้วมา discuss มันก็เท่ากับมี bias เห็นๆ เพราะในตอนแรกผมบอกได้แค่ว่าไม่ใช่ lymphoma เป็น แบบdisseminated infection บางอย่างที่ไม่มีปัญญาบอกว่าเป็นตัวอะไรกันแน่ จนกระทั้งรู้ final diagnosis ผมชอบอยู่โรงเรียนแพทย์อย่างจุฬาก็เพราะมี case สวยๆ rare ๆ แบบนี้ให้ได้เรียนรู้และ exercise brain ตลอดเวลาก็ต้องขอบคณอาจารย์ชุษณาที่ทุ่มเทกับ CPC ตลอดมา

เห็นด้วยครับ
เกิด case นี้ เผลอหลุดไปอยู่บางสถาบัน อาจจะถูก Dx เป็น PM + RA แล้วก็กลายเป็น overlap syndrome
โดน steroid แล้วตายฟรี (ไม่ได้พาดพิงถึงใครนะครับ :-\)
และเห็นด้วยครับ ว่าจุฬามี case สวยๆมากๆ
แต่ความจริง case สวย อาจจะไปอยู่ทุกที่ เพียงแต่จะมีใครพยายามหาคำตอบที่แท้จริงออกมาหรือเปล่า หรือพอใจกับ Dx อะไรง่ายๆ ก็เลยกลายเป็น case ไม่สวย (แท้จริง Dx ผิด)

ว่าแต่ idea เอา อ ชุษ ขึ้น CPC ก็น่าสนใจดี อยากดูช่วงเอาคืนมั่ง  ;D
สุดท้าย ก็มีแต่คนอุบว่า ไอ้ตัวประหลาดนี้ ตกลงมันเป็นตัวอะไร

1. Neurological abnormalities
    1.1  Tongue fasciculation, low-back pain radiated to coccyx, generalized areflaxia, asymmetry and non-simultaneous muscle weakness, very course of about > 1 year, and proteinorrhachia favor nerve roots; and hence lymphoma > TB/NTM/fungal (Crypto, Histoplasma, or Penicilliosis) infection >>> multicentric localized infection.
          Eventually these are not confirmed by electrophysiologic study.
    1.2 Neck muscle weakness, mechanical dysphagia, tender muscles, and marked elevated muscle enzymes favor myopathy;  and hence multicentric localized infection > lymphoma > TB/NTM/fungal.
          Eventually these are confirmed by electrophysiologic study.
2. The whole left leg swelling favors DVT; and hence lymphoma > multicentric localized infection >>> TB/NTM/fungal infection.
          Eventually it was severe muscle swelling.
3. Significant and marked weight loss favors lymphoma, TB/NTM/fungal infection > multicentric localized infection.
4. Visceral lymphadenopathy favor TB/NTM/fungal infection, lymphoma > multicentric localized infection.
 Therefore, initially before obtaining all investigation results, I very much concerned lymphoma.

The first one who had suspicion of this microorganism is Arjan Worapot krub, not me.

Very much for the very delayed response!?!
I think every one can follow the VDO.

The final Dx is disseminated microsporidiosis, likely the new species since its sequence doesn't match >95% identity in comparison with the other known species especially in humans. The phylogenetic tree reveals that Cystosporogenes and Endoreticulatus are close species with this new species. Both species can be isolated from the insects for example Cystosporogenes is from the bronze birch borer, a kind of insect in North America. I wonder this CPC patient probably got the pathogen from ingestion of insects??

I have to thank both Arjans Somchai and Chaturong of Parasitology Department for their effort to identify this emerging Microsporidium in our CPC patient; unluckily the microorganism from the 24-hour urine specimen couldn't be isolated by cell culture technique due to cytotoxic effect from contaminated Candida. But they would be able to sequence this pathogen from the bone marrow specimen which turns out to be the new species.
I learn very much from this case, and wonder that the microsporidiosis, an interesting emerging infectious disease, is underestimated all over the world due to unrecognition and very broad host range varying from insects, reptiles, fish, birds, and mammals.

Dear all:
FYI, the recognition of the microsporidia in the slides is based on the belt-like structure within the cytoplasm of the cell which varies from 1-4 micron; you all can note it from the bone marrow aspirate slide from the CPC protocol.
