Dear
Which one, microsporidiosis vs toxoplasmosis vs leishmaniasis vs sarcocystosis vs penicilliosis vs histoplasmosis, could be associated with pulmonary HT and more than 1-year course?????
And from the picture, how can you differentiate all of these microorganisms from each other????

Actually, toxo and sarcocystis should be longer than these. And tends to form cysts or pseudocysts respectively. Leishmania and trypanosomes should see some flagellate form in the blood. Penicillium should see some septate division and histo should budding and have pseudocapsule. I never come across the characteristic of microsporidia in h and e or giemsa, it is well known for modified afb and em. I guess the teardrop like and dark organelles at the pole like the EM structure shown like this case.
Toxo tend to be acute.
Histo can explain most of the features in this case.
But if it is not the case, microsporidium might be the answer.
Pulmonary manifestation is not well characterized in these rare, opportunistic infection, except in histo.

P Nammon, our former internal medicine resident and now chest specialist with multiple fellowship said that any organisms that can filter pass the microcirluation has a potential role in causing obstructive vasculopathy and then pulmonary hypertension.

Well, actually I believe parasites that are commonly known to cause pulmonary hypertension are schistosomes and sometime filarias ka, but apparently not the cause for this person. and I don't have that many boards na ya!

เพิ่งกลับมาจาก Paris แบบ backpacker มา
กลับมาถึง ก็ได้กลิ่นธูปโชยมา
ก็เลยเปิดกระทู้  งานนี้ กลายเป็น อ คนไกลบ้าน ลุยเดี่ยว
ก็เลยมาขอ featuring ด้วย

ก่อนอื่น ขอชื่นชมคนเขียน protocol มากๆว่า
เขียนได้ราบเรียบดีมาก ไม่มีจุดสนใจอะไรเลย
อ่านแบบหลุดๆได้เลยนะเนี่ย
แต่จริงๆซ่อนอะไรไว้เยอะ

อ่านไป ก็นั่งคิดว่า
ไอ้ lab บางอย่างนี่ ส่งไปทำไม
แต่พออ่านจบ ก็ถึงบางอ้อ ว่า
เขียนแบบเนิบๆ ไปเรื่อยๆ เรียงตามชนิด Lab
แต่ไม่เรียงตามลำดับการส่ง Lab ที่ควรจะเป็น
กว่าจะปะติดปะต่อได้

ทาง neuro น่าจะเป็น infectious myositis
แต่มีอะไรแปลกนิดๆตรง reflex 0  กับ tongue มี fasciculation
ยังไงก็ตาม ก็ยังเอา muscle disease อยู่ดี

case นี้ มี organ ที่ involve หลักๆคือ muscle กับ BM
muscle เป็น necrotizing granuloma  น่าจะเรื่องเดียวกับไอ้ตัวใน BM
แต่ที่ BM ดันไม่มี reaction พวก granuloma  แปลว่ามันเด่นที่เข้าเลือดหรือ dissemination โดยไม่มี reaction ใน marrow
โดย morphology ผมไม่รู้ครับ ว่า yeast หรือ parasite พวก protozoa
ก็เลยพยายามอาศัยใช้แต่ clinical ก็พบว่าจอดสนิท
protozoa ทุกตัว มันให้ feature ของ muscle และ disseminated ได้
แถมหน้าตาก็ "คล้าย" กัน ซะอีก ไม่ว่าจะ Toxoplasma, leishmania, tripanosome (ไอ้ตอนมันกลมๆนะ แต่ตอนที่มันยืดยาวแล้ว มันแยกได้)
ก็เลยต้องหาตัวช่วย
ไปหาแผนก clinical micro  สรุป จอดสนิท
Cyto สรุป ไม่แน่ใจ ได้ microsporidium เพราะเหมือนจะมี barๆ  แต่ไม่ชัด
Patho ทำให้แผนกทะเลาะกัน

เอาเป็นว่าผมหาตัวช่วยไปๆมาๆ ก็ได้คำตอบแล้วโดยบังเอิญ
ดังนั้น ก็เผ่นชะแว้บดีกว่า

อ้อ ไอ้พวก CD มาเนียนๆกับ lab marrow  แต่จริงๆจะหาภาวะ immmune def ใช่หรือเปล่าครับ
คนนี้เสียที่ T cell  ประมาณ number หรือเปล่า
ไม่มีความรู้เรื่อง immune def ด้วยสิ

อีกนิด ถ้าจำไม่ผิด อ ธีระวัฒน์ เคย Dx Leishmania ในคนไทย
แต่เสียใจด้วย
case นี้ ไม่ใช่ case นั้น

Clue ของมันคือ Bar ตรงที่ mark ลูกศรแดงไว้ครับ

ของคนไข้รายนี้ พอซูมดีๆ ก็เหมือนมีนะครับ

ว่า ทำไม case นี้ถึงไปทำ Bone marrow ล่ะครับ

ID doctor didn't take care this patient at the first time. I think the primary physicians had a high suspicion of NHL due to proximal muscle weakness probably caused by CSF infiltration of the cauda equina plus/minus myositis,  suspicious DVT of left leg, significant weight loss and fever.
Aj DK, would you mind to teach us regarding the cause and diagnostic approach for pulmonary HT from the given data???  And how you can correlate with other abnormalities???

ผมไม่ใช่ expert เกิดมาก็ไม่เคยเจอไอ้ตัวนี้เลย
เอาเป็นว่า จะหาอ่านและมาเล่าสู่กันฟังละกัน
งั้น...ขอแลกกับคำถามนี้ที่จะเป็นประโยชน์กับ Resident
เพราะมันเป็นคำถามที่เกิดเมื่อผมเห็นหน้าไอ้ตัวนี้ว่า
จาก morphology จะแยกได้จาก yeast ทั้ง histo, penicillium อย่างไร
และจะแยกจากเพื่อนๆมันในกลุ่ม protozoa ที่ทำ อ คนไกลบ้าน DDx เป็นกองทัพ ได้อย่างไร
รวมถึงต้องย้อมอะไรด้วย
(เพราะคำถามนี้ เล่นเอาผมวิ่งวุ่นและตอนนี้ก็ยังสะเปะสะปะอยู่)

Dear all:
From BMA clues to exclude yeasts (Histoplasma, Penillium, Crypto, other yeasts) and protozoa (Leishmania, toxoplasma, sarcocystis, microsporidia etc)
1. Ovoid shape
2. 2-3 micron in size
3. No intracellular form, just free microorganisms
4. No budding, no binary fission
5. No kinetoplast perpendicular to a nucleus inside the cells
6. Only one island of microorganism in bone marrow aspiration specimen
7. No microorganisms found in BM Bx
8. Unique feature inside each cell; this is a valuable clue!!!???!!!

Dear all:
BM Bx specimens usually give smaller size of cells than BM aspiration due to processing step, I think! This kind of microorganism is about 2 micron in size, you can guess the explanation for the dissociation between them??!!??

Hemato ได้รับ consult เพราะว่ามีคนสงสัย lymphoma ตอนไปดู case มี clues บางอย่างที่อาจทำให้สงสัย lymphoma ได้เช่น prolonged fever, weight loss, mild splenomegaly, small lymphadenopathies from imaging, and high LDH (ซึ่งก็เป็นจาก myositis ก็ได้) ส่วน DVT นั้นผมทราบตอนไปดู case ว่าไม่ใช่จาก imaging แล้ว แต่ disseminated infection โดยเฉพาะ cryptococcosis และ histoplasmosis ก็ยังอยู่ใน DDx ของผมอยู่ในตอนแรก เลยตัดสินใจให้ทำ BM study เมื่อดูจาก bone marrow aspiration แล้วไม่ได้สงสัย lymphoma เลย ตอนเจอ organism ที่ว่าก็คิดว่าน่าจะอธิบายได้ทั้งหมดจาก organism ที่ว่านี้ได้โดยตอนนั้นยังไม่รู้ว่ามันคือตัวอะไรกันแน่

ทาง ID ต่างหากที่ยังสงสัยว่าคนไข้มี underlying เป็น lymphoma ไปสั่งย้อม very extensive immunohistochemistry เพื่อพยายามหา lymphoma เองทั้งหมด ผมก็เคยพูดใน unit round แล้วว่าไม่สงสัย lymphoma เพราะ clinical มันเป็น aggressive lymphoma ซึ่ง course ที่ยาวกว่า 1 ปี มันนานเกินไป และยังคุยกับ chief ward เลยว่าน่าจะเป็นไอ้ตัวนี้ทั้งหมดนั่นแหละ ทาง ID ต่างหากที่ยังไม่ยอมหยุดจะยัดเยียดให้เป็น lymphoma ให้ได้ จน investigate หา lymphoma จนสุดทางแล้วถึงจะยอมรับว่าไม่ใช่ lymphoma แล้วค่อยพยายามมาอธิบาย clinical syndromes ทั้งหมดจากเชื้อตัวนี้
 
ผมถึงไม่เข้ามา response ตั้งแต่แรกเพราะรู้ว่า case นี้มันยากมากๆ เพราะจริงๆ ผู้เกี่ยวข้องใน case นี้ทั้งหมด ไม่ว่า Neuro, Hemato, Pulmo, ID, Rheumato ไม่มีใครสามารถอธิบาย clinical syndrome ของคนไข้ได้ทั้งหมดตั้งแต่ต้น (จริงๆตอนเจอตัวประหลาดนี้ผมยังบอก resident hemato เลยว่า ดูว่าใครรับ consult ที่ตึกนี้ถ้าไม่ใช่อาจารย์ชุษณา ผมจะให้ fellow ID ปิด case นี้ไว้แล้วชงอาจารย์วันล่าให้ อาจารย์ชุษณาเป็นคนทำ แต่ก็ผิดแผนเพราะอาจารย์ชุษณารับ consult ตึกนั้นพอดี)

จริงๆถ้าไม่ถูกพาดพิงก็คงไม่เข้ามา response เพราะรู้ case แล้วมา discuss มันก็เท่ากับมี bias เห็นๆ เพราะในตอนแรกผมบอกได้แค่ว่าไม่ใช่ lymphoma เป็น แบบdisseminated infection บางอย่างที่ไม่มีปัญญาบอกว่าเป็นตัวอะไรกันแน่ จนกระทั้งรู้ final diagnosis ผมชอบอยู่โรงเรียนแพทย์อย่างจุฬาก็เพราะมี case สวยๆ rare ๆ แบบนี้ให้ได้เรียนรู้และ exercise brain ตลอดเวลาก็ต้องขอบคณอาจารย์ชุษณาที่ทุ่มเทกับ CPC ตลอดมา
